Osaka has a crowded field of live sumo shows and they are not the same product. Here are the ones with reviews, compared on price, length, food, how much you do yourself and how flexible the booking is.

Basics
A sumo show in Osaka is not one product. The listings fall into four types. Studio sessions are small rooms with a clay ring, a host and two retired wrestlers, and the show this site sells is one of them: it meets at Sumo Studio Osaka in Nishinari-ku, runs about 1.5 hours and takes groups of up to 53. A second group of studio sessions starts near JR Imamiya Station and includes an ex-pro wrestlers show and a breakfast show.
The second type is the theater. THE SUMO HALL HIRAKUZA OSAKA, on the 8th floor of Namba Parks, was built for the purpose and seats 180, and its listing counts 2 hours for the bento, the show and the photo.1 The third type is the dinner show, where the meal is the center of the evening. A restaurant by Rinku Town Station near Kansai International Airport runs a hot-pot show, and a Wagyu sukiyaki version offers guaranteed front-row seats.
The fourth type is the morning practice, which is a different thing and an uncommon one in Osaka. A listing for a morning practice in Daito city describes an authentic training session for a small group that meets at Active Square Daito, and has no reviews yet. The national tourism organization says drop-in stable visits are discouraged, though some agencies arrange small-group tours.2

Prices start low. Two GetYourGuide listings at the studio near Hanazonocho Station start from $41, one as a sumo experience with a live show and audience challenge and one as a show with a live match and a photo. The ex-pro wrestlers session starts from $44, and the breakfast show from $45.
The mid-range holds the show this page recommends and the hot-pot show. Our Viator listing starts from $62 and the hot-pot show with a kimono corner from $65. A show in an ukiyo-e inspired venue starts from $76, though it has only 3 reviews so far.
The top of the range is the hall and the dinner shows. The hall show with a bento starts from $92, a small-group show limited to 10 participants from $121, and the front-row Wagyu sukiyaki show from $127. Every figure is a starting price, so the amount you pay depends on the date and the group, and the price you see when you pick a date is the one that counts. Compare like with like: a show with a meal and a show without one are priced for different evenings.
Reviews on file
Review counts as each listing states them; ours is a Viator count, the others GetYourGuide.
Most shows fit into one sitting. Our studio session lasts 1.5 hours, the breakfast show 1.5 hours, and the ukiyo-e show 1 hour. The hall counts 2 hours with the meal and the Wagyu show 2.5 hours, while the hot-pot listing states a range of 1.5 to 4 hours depending on the start time.
Start times spread across the day. The breakfast show begins at 8:30 AM, the hot-pot restaurant runs a lunch show at 1:00 PM and an evening show at 6:00 PM, and the hall lists a Workshop Experience at 13:30 and Sumo Show slots at 17:00 and 20:00. The timeline on this page lays them out in order.
That spread is useful for planning. A traveler who lands in the morning can start with the breakfast show, an afternoon visitor has two options after lunch, and an evening slot works for dinner plans. Build a margin for the walk and the lift into your plan, whichever slot you choose.

Front-row seating
Three of the shows include food. The hall ticket comes with a bento or a snack, the breakfast show with a sumo-style breakfast, and the hot-pot and Wagyu shows build the meal into the evening. Chanko nabe at the hot-pot restaurant is optional, and meals are not included unless a food plan is chosen.
Dietary needs matter. The hall's tourism-bureau page says vegan and halal bento options are available, and the Wagyu listing mentions a halal Wagyu sukiyaki option.4 The studio shows are about the sumo rather than the meal, and the record for our session lists no meal, so eat before or after. One reviewer recommended the supermarket across the road for lunch.
If food is the point of your evening, the dinner shows are the ones to weigh. If food is only a bonus, the hall's bento keeps the price down and the show tight. And if you are watching your budget, a studio show plus a meal of your own choice often works out cheaper than a dinner show, though your own tastes decide.

Nearly every show lets someone into the ring, and the rules for who differ. At the hall, three winners are chosen by lottery to step in and face a professional wrestler.3 At our studio a limited number of spots let guests step in, and the hot-pot restaurant lets guests train in the ring after the show, with one-on-one matches assigned at random if more guests want to than the ring allows.
The breakfast show promises a chance to wear a mawashi and step into the ring yourself, and the ex-pro wrestlers session says guests can step onto the ring and try sumo. The studio near Hanazonocho Station also offers a one-round match between retired wrestlers, which is the part to watch if you do not want to volunteer.
If you want to join, choose a show with a clear route into the ring. If you want to watch, the hall is the safer pick, since the lottery is only three places in the audience. Neither type of show is a tournament: the studio listing itself says it is an entertainment show performed by retired wrestlers.

Checklist
The refund rule splits the field. The hall listing is non-refundable, while the studio, the hot-pot, the breakfast and the Wagyu listings offer free cancellation up to 24 hours before. If your plans are fixed, that difference does not matter. If you might move a day, it is a deciding factor.
On access, the GetYourGuide listings for the studio, the hall, the breakfast show and the Wagyu show carry a wheelchair accessible label. Confirm access with the provider for your own needs before you pay.
For most visitors the choice comes down to a few questions: how much to spend, whether to eat, whether to volunteer and whether the plan may change. Ask for the best sumo show in Osaka and the honest answer is the one that fits those questions. Our pick is the studio show with the largest number of reviews and free cancellation. The quiz at the top of the page turns those questions into a recommendation in two taps.
